About the Event
In collaboration with the State of Illinois, the Elk Grove Village Public Library will host a COVID-19 booster clinic in the Large Meeting Room at the Library on Monday, January 17, 2022 from 12pm to 4pm.
This event is for booster shots only. It is free and open to the public to all individuals ages 12 years and up who have been fully vaccinated with the Johnson & Johnson, Moderna, or Pfizer vaccine and are eligible to receive a booster shot. All three types of boosters will be available.

On the Day of the Event
Each patient will be required to fill out an information form, so please bring a photo ID, an insurance card (if available), and your vaccination card with you.
Licensed clinicians contracted with the Illinois Department of Public Health will administer booster shots. An area will be designated to each patient for 10-15 minutes to observe any adverse reactions after booster shots have been administered. If you experience any type of side effect, please report it to a clinic volunteer.
